WHO IS ON MY PALLIATIVE CARE TEAM?

Our interdisciplinary palliative care team is dedicated to providing compassionate, comprehensive care for patients and support for their families. We help manage physical symptoms while also addressing emotional and spiritual concerns. We support families in navigating complex medical decisions and honoring each patient’s wishes for care.

Medical Director

Leads the interdisciplinary team in developing and overseeing each patient’s personalized care plan.

Skilled Nursing

Nurse practitioners coordinate care, provide symptom management, and educate patients and families about treatment options and comfort strategies.

Social Worker

Assesses emotional, social, spiritual, and financial needs, offering counseling and connecting families with helpful resources.

Chaplain

Provides spiritual support, counseling, and bereavement care tailored to each patient’s beliefs and values.

Compassionate Care, Personalized Support

At The Care Team, our passionate team of specialists develop a care plan entirely on your individual wants and needs. We work closely with you and your healthcare team to provide coordinated plan of care and an extra layer of support when you need it most.

Who is eligible to for palliative care?

Palliative care is available for patients with serious or life-limiting illnesses that affect daily life or cause ongoing symptoms or pain. Examples include cancer, chronic obstructive pulmonary disease (COPD), congestive heart failure, and Parkinson’s disease.

Do I need an order for palliative care?

Yes. A physician’s order is required. Our palliative care team will work closely with your doctor to create a care plan that supports your comfort and quality of life.

How does palliative care work with my current treatment?

Palliative care complements the treatment you are already receiving. Our team provides in-home support, monitors symptoms, and helps reduce emergency room visits and hospital stays. We focus on physical comfort while also offering emotional and spiritual support to patients and caregivers.

What is the difference between palliative care and hospice care?

Palliative care can be provided at any stage of serious illness and alongside curative treatments. Hospice care begins when curative treatment is no longer pursued and focuses entirely on comfort at the end of life. Both provide compassionate support, but the timing and goals of care are different.

Where is palliative care provided?

Palliative care is traditionally offered in one of three settings: at home,  in a hospital or specialized clinic, or in a nursing home. Your care team, along with your family, can help you make the best decision on where to receive treatment depending on your needs.

Is palliative care covered by my insurance?

Palliative care services are covered by Medicare Part B and many private insurance plans. Accepted insurances include:
