WHO IS ON MY HOSPICE TEAM?
Our caring and expert staff includes specialists who collaborate to care holistically for you and your family.
Medical Director
Leads our interdisciplinary team to develop the holistic care plan.
Attending Physician
Certifies patient’s diagnosis, assesses patient’s needs and directs the plan of care together with the hospice team.
Registered Nurse
Coordinates the team to support and educate the patient and family, and provides pain and symptom management.
Home Health Aide
Provides direct personal care to the patient and emotional support to the entire family.
Therapists
Provide support and care through specialized therapies including massage, pet therapy, music therapy, aroma therapy and more.
Social Worker
Assesses the family’s emotional, social, spiritual and financial needs to provide counseling and connection to support resources.
Volunteers
Provide companionship to the patient and family, assist with errands and provide respite time.
Chaplain
Assesses patient and family’s spiritual needs, provides direct counseling and bereavement support.
Our Care Centers on the Individual
We take time to learn what is most important to the patient. Our team has enabled trips to concerts, casinos and cabins. We’ve helped patients enjoy food, family and friends. When the conversation starts with what do you really want, it can spark any number of experiences. Ask us what’s possible.

Support in Bereavement
We’re here to support families after the loss of a loved one. We offer much needed services like grief counseling, assistance with memorial preparations, referrals to community agencies and timely messages of support and positivity.

Do I Qualify?
Yes, when the hospice physician and the patient’s attending physician certify the patient is eligible.
Yes, when the patient chooses a palliative approach to treatment rather than a curative approach to a life-limiting condition.
How Will I Pay?
Hospice care is fully covered by Medicare.
Medicare coverage includes all hospice services, medications and most supplies and equipment.
Hospice care is also covered by most private health care insurances.
