The Care Team, LifeCare Home Health Family, VitalCaring Group, and Mission Healthcare will bolster care access in their respective markets as demand for services increases
FARMINGTON HILLS, Michigan, Dec. 3, 2025 – Today, four leaders in regional hospice and home health services, The Care Team, LifeCare Home Health Family, VitalCaring Group, and Mission Healthcare announced the successful acquisition of Traditions Health, a hospice, palliative, and home health provider based in Tennessee with operations in 18 states.
The transaction has been carefully structured to ensure each provider’s proven expertise and geographic footprints match with acquired geographies of Traditions Health to create value for patients and communities
- The Care Team has acquired hospice and palliative care markets of Traditions Health in Illinois, Indiana, Ohio, and Virginia.
- VitalCaring Group has acquired home health, hospice, and palliative operations in Alabama, Arizona, Kansas, Louisiana, Missouri, Mississippi, Nebraska, Nevada, Oklahoma, and Texas.
- LifeCare Home Health Family has acquired hospice and palliative operations in Georgia.
- Mission Healthcare has acquired hospice and palliative operations in California and Oregon.
