What Is Home Health Care?
Home Health Care is specialized care and support services for patients recovering from illness, injury, or a hospital stay. It can also benefit someone who may be chronically or terminally ill. Home Health Care can help keep you on track when the goal is restored health, managing symptoms, or rehabilitation. The Care Team is here to help with medical, nursing, therapeutic treatments and daily living activities.

RN Case Manager
Leads the team and coordinates with your physician about your care plan.
Skilled Nurses
Provide symptom and disease management, patient education and other specialized care.
Medical Social Workers
Help connect you and your caregivers to resources in the community to aid in your recovery.
Occupational Therapists
Support your recovery and regain independence in self-care activities, such as bathing and grooming.
Speech Therapists
Help you with speech, cognitive therapy, swallowing and eating difficulties.
Physical Therapists
Assist you with walking, exercising, and learning to use new mobility equipment.
Dietitians
Support your overall health with personalized nutrition education and dietary plans.
Home Health Aids
Provide direct personal care including bathing and grooming.
Patients Typically Receive Home Health Care:
- When they have a doctor’s order
- When they have homebound status
In addition to physical and occupational therapies, patients may benefit from disease and pain management, cardiac care, wound care, emotional wellness, medication teaching and management. Our services are focused on your specific needs.

Do I Qualify?
Yes, when you have a doctor’s order for home health services.
Medicare defines eligibility requirements for home health benefits. If leaving home is difficult, we can talk with your doctor and assess your eligibility.
How Will I Pay?
Home health care is a Medicare Part A benefit that does not require a copay or deductible for patients who meet eligibility criteria.
Other payment sources may include Medicaid, private insurance, workers’ compensation, private payment or alternative funding.
