What Is Home Health Care?
Home Health Care is specialized care and support services for patients recovering from illness, injury, or a hospital stay. It can also benefit someone who may be chronically or terminally ill. Home Health Care can help keep you on track when the goal is restored health, managing symptoms, or rehabilitation. The Care Team is here to help with medical, nursing, therapeutic treatments and daily living activities.
Patients Typically Receive Home Health Care:
- When they have a doctor’s order
- When they have homebound status
In addition to physical and occupational therapies, patients may benefit from disease and pain management, cardiac care, wound care, emotional wellness, medication teaching and management. Our services are focused on your specific needs.

RN Case Manager
Leads the team and coordinates with your physician about your care plan.

Support your recovery and regain independence in self-care activities, such as bathing and grooming.

Do I Qualify?
Yes, when you have a doctor’s order for skilled home health services.
Medicare defines eligibility requirements for home health benefits. If leaving home is difficult, we can talk with your doctor to asses your eligibility for skilled services.
How Will I Pay?
Home health care is a Medicare Part A benefit that does not require a copay or deductible for patients who meet eligibility criteria.
Other payment sources may include private insurance, workers’ compensation, private payment or alternative funding.
